1. Please only make one submission. Multiple requests will be discarded.
2. Check accuracy before submitting application
3. Type in NAME exactly how you would like it to appear on the certificate (including capitalizations, hyphenations, etc)
4. Applications are processed once a month. If a deficiency is found (failed lesson, non-payment, etc) you will be notified by email. You will then need to correct the deficiency and re-apply for graduation the next available month. Please ensure that you have completed all graduation requirements BEFORE you complete this application.
